  1. Submit complete documentary requirements to Migrant Workers Office (MWO) (formerly POLO) Tokyo through post mail:

CHECKLIST OF REQUIREMENTS A & B
【A】Changed Employer Onsite / Without Existing Records or Not Registered with DMW
  1. Duly accomplished DMW Standard Employment Contract
    => For template, DMW CONTRACT FOR PROFESSIONAL & SKILLED WORKERS

  2. Copy of passport valid for at least six (6) months from the date of intended departure
    => フィリピン人雇用者のパスポートの写し
  3. Copy of valid and appropriate visa or residence card
    => 在留カードの写し
  4. Any proof of existing employment with the current employer to which the worker is resuming employment such as a current certificate of employment, valid ID, or recent pay slip
    => 在職証明書又は給料明細書
  5. Sworn statement of the worker providing an explanation of how he/she was hired by the current employer.
    =>SWORN STATEMENT
  6. Copy of the Hokensho (Insurance Card) provided to the worker
    => 健康保険証の写し

  7. Copy of valid passport of the authorized representative who signed the employment contract or valid ID with English translation
    => 受入機関/雇用主のパスポートの写し
  8. a. For Companies: Company Registration with English translation
    b. For Sole Proprietorship : Tax Certificate with English Translation
    => a. 法人の場合: 登記簿謄本「履歴事項証明書」及びその翻訳
    => b. 個人事業の場合: 納税証明書・納税申告書の写し及びその翻訳
  9. Other relevant documents as may be required.
    FOR CONSULAR MISSIONS: Provide photocopy of the required documents (1 set)

NOTE: Kindly also apply for OWWA Membership. It is a separate application from contract verification.

BReturning to a previous (same) employer but WITH GAP/INTERRUPTION in the duration of employment service with that employer
