  1. Submit complete documentary requirements to Migrant Workers Office (MWO) (formerly POLO) Tokyo through post mail:

  2. Attach a return letter pack (Japan Post)

CHECKLIST OF REQUIREMENTS A & B
【A】Changed Employer Onsite / Without Existing Records or Not Registered with DMW / Change Jobsite
  1. Duly accomplished DMW Standard Employment Contract
    => For the template, Click Here HSW Standard Employment Contract
  2. Copy of passport valid for at least six (6) months from the date of intended departure (WORKER)
  3. Copy of valid and appropriate visa or residence card of WORKER
  4. Any proof of existing employment with the current employer to which the worker is resuming employment such as a current certificate of employment or recent pay slip of the WORKER
  5. Sworn statement of the WORKER providing an explanation of how he/she was hired by the current employer

  6. Copy of the Hokensho (Insurance Card) provided to the WORKER
  7. Copy of valid passport AND Residence Card/Gaimusho ID of EMPLOYER
  8. Original Employment Certificate of the Employer with English Translation of EMPLOYER (Must be issued by the Company or Embassy)
  9. Copy of Letter of Guarantee submitted to the Immigration of Japan
【B】Returning to a previous (same) employer but with gap/interruption in the duration of employment service with that employer
  1. Duly accomplished DMW Standard Employment Contract

  2. Copy of passport valid for at least six (6) months from the date of intended departure (WORKER)
  3. Copy of valid and appropriate visa or residence card
  4. Any proof of existing employment with the current employer to which the worker is resuming employment such as a current certificate of employment, valid ID, or recent pay slip
  5. Copy of previously issued OEC or verified employment contract by POLO/MWO
  6. Copy of the Hokensho (Insurance Card) provided to the worker
  7. Copy of valid passport AND Residence Card/Gaimusho ID of EMPLOYER
  8. Original Employment Certificate of the Employer with English Translation (Must be issued by the Company or Embassy)
  9. Copy of Letter of Guarantee submitted to the Immigration of Japan
