OWWA MEMBERSHIP REQUIREMENTS

A. ELIGIBILITY

OFWs with valid overseas employment and work visa, and are processed at DMW
OFW who have acquired regular/permanent residency (Permanent or Long-Term Visa holder)
OFWs who have left the country without proper documentation as OFW from DMW and have acquired valid work permit and visa in Japan

B. DOCUMENTARY REQUIREMENTS

Duly filled-up and signed OWWA OFW Information Sheet
=> FOR OWWA MEMBERSHIP: INFORMATION SHEET
Copy of proof of valid employment (with English translation)
- FOR RENEWAL WITH THE SAME EMPLOYER: Copy of valid Employment Contract or Certificate of Employment with hanko of employer or authorized company representative issued WITHIN A MONTH of Membership Application
- FOR RENEWAL WITH NEW/CHANGED EMPLOYER: Copy of valid Employment Contract (with English translation)
- FOR NEW ENROLEE/MEMBER: (a.) Copy of Employment Contract with English Translation and (b.) Filled-out Declaration of Intent to be an OWWA Member Form
Copy of valid Philippine Passport bearing photo, name, and signature
Copy of valid Residence Card
- For Official/During Mission Visa: Copy of valid visa stamp in passport
Proof of existing Company/EmployerFOR RENEWAL WITH THE SAME EMPLOYER:
- For Company: Copy of Company Registration
- For Sole Proprietorship/Individual: Copy of valid ID (Residence card or Gaimusho ID) of Employer
